The Human Services Department provides a variety of courses and programs that have been structured to promote strong links between the programs, field placement agencies and community partners.

MEDICAL CLEARANCE is required for all clinical/field placement courses and courses with a clinical component.

This form must be submitted to the HEALTH SERVICES OFFICE, IAHS - ROOM 303 accompanied by a one-time processing fee. Cheques should be made payable to “Mohawk College Health Services” with your student ID number clearly visible on the front of the cheque. Cash will be accepted if dropped off in person to Room 303 at the IAHS.
