ONLY TO BE COMPLETED BY APPRENTICES WHO HAVE RECEIVED AN OFFER OF CLASSROOM TRAINING FROM THE MINISTRY OF TRAINING, COLLEGES AND UNIVERSITIES.

IF YOU HAVE ALREADY PAID YOUR FEES PLEASE DISREGARD THIS LETTER.

Dear Apprenticeship Student:

Please complete the Apprenticeship Fee Payment Form (see links below) and return with your payment as soon as possible to reserve your seat. Once you pay your fees, including $5.00 for the Student I.D. Card, you will be sent additional information regarding registration date and time as well as a receipt of your payment.

Please Note: If you wish to rent a locker, please include the $5.00 fee with your payment.

Please note the following methods of payment:

1. Cash/Interac:
   To be paid in person only.
   
   
2. Certified Cheque/Money Order:
   • To be paid either in-person or mailed.
   • To be made payable to Mohawk College.
   
   
3. VISA/MASTERCARD:
   • Please record your VISA/MASTERCARD number and expiry date on the Fee Payment Form, include your signature and mail the completed form, or
   • Pay in-person, or
   • Fax to (905) 575-2544

(Please Note: We are unable to confirm receipt of your fax. Faxing multiple times could lead to multiple billings)
